The information on this form is used to calculate your 2024 Enrolment Notice. By completing and submitting this form, you agree to the terms and conditions outlined in the "Confidential information and participant consent" section of the 2024 AgriStability Information Guide for New and rejoining participants.

How to download and open a PDF form

    1. Right-click on the link you want to save

    3. Choose the location on your computer where you would like to save the file

    8. Go to the location where you saved the file

    3. Choose the location on your computer where you would like to save the file. Your computer will start the download once you have selected a location.

    5. Go to the location where you saved the file
    6. Select the PDF file
    7. Select "Open"

Corporation information (if applicable)

Enter the name and Participant identification number (PIN) of shareholders who have controlling interest in the corporation. Check the box if the shareholder is enrolled in AgriStability as an individual for the 2023 program year and has transferred land or inventory to the corporation under Section 85 of the Income Tax Act to the corporation for the 2024 program year. Submit a copy of the signed document showing that a legal transfer to the corporation has taken place.
